At Trafalgar Medical Group Practice, we offer a comprehensive range of private medical services outside of NHS provision. These services cater to specific medical needs such as DVLA medicals, adoption medicals, and sports medicals. Below, you can find detailed information on each service we provide, along with pricing.

We also offer most Sports Medicals for non-practice patients with Dr. J Collins, including those for Marathons, Triathlons, and Ultra-runs. Please contact the practice for more details. You can find the full list of prices here.

Adoption & Fostering AH & AH2 (£84.00 Each)

This service involves completing the AH and AH2 forms required for adoption and fostering applications. It ensures that the prospective parents are in good health and able to care for a child. This process typically involves a review of medical history and a physical examination to meet adoption agency or fostering panel requirements.

Army Medical Questionnaire (£50.00)

A medical questionnaire required for individuals applying to join the British Army. This document assesses general health, past medical conditions, and overall fitness. The GP will complete the form based on medical history and possibly a physical examination to ensure fitness standards are met.

Boxing BCR1 Phase 1 (TMGP Patients Only) (£75.00)

A pre-competition medical examination required for amateur boxers. The BCR1 ensures that individuals are fit to participate in boxing events. The GP will assess overall physical fitness, heart health, and other factors critical to safe boxing participation. This service is only available to Trafalgar Medical Group Practice patients.

Camp America F2F (£75.00)

A face-to-face medical assessment required for applicants to the Camp America programme. This assessment confirms that applicants are healthy enough to work with children in outdoor and physically demanding environments. It includes a medical history review and a physical examination to ensure fitness for the programme.

Seatbelt Exemption Certificate (£40.00)

A medical certificate confirming exemption from wearing a seatbelt due to a health condition that makes it unsafe or uncomfortable. The GP assesses the patient’s condition and issues a certificate if they meet the legal requirements for seatbelt exemption, a service often requested by the DVLA.

Comprehensive Report (Full) (£95.00)

A full medical report providing a detailed summary of a patient’s medical history. This report is often required for legal, insurance, or employment purposes and includes an overview of the patient’s health status based on their medical records.

Comprehensive Report (Full, Including Medical) (£150.00)

This report includes both a comprehensive review of the patient’s medical history and a physical examination. It is often required for insurance claims or legal cases where a detailed health assessment is needed.

Computerised Records (Free)

Patients can request a free copy of their computerised medical records. These records include information on past treatments, prescriptions, and consultations. A small administrative fee may be charged for repeat requests.

Disabled Parking Bay Application (£30.00)

A medical form for those applying for a disabled parking bay near their home. The GP will assess the patient’s mobility issues and overall health to determine if they qualify for this parking bay based on local council requirements.

Diving Medical (Sport/Commercial) (£160.00)

A medical examination for those wishing to engage in either recreational or commercial diving. This check ensures that the patient is fit to dive safely, with a focus on respiratory and cardiovascular health, as well as overall physical fitness.

DNA 1, 2 & 3 Samples (£55.00 Each)

DNA sample collection for legal or personal reasons such as paternity tests or other genetic analysis. Each sample incurs a charge of £55.00. The samples will be handled in compliance with legal standards for DNA testing.

Driving License Form (£50.00)

Completion of medical forms required by the DVLA for driving license applications or renewals. This is especially important for those with certain health conditions. The GP will assess fitness to drive based on medical history and possibly a physical examination.

Education Certificate (Fit/Unfit), Including School Fees Insurance Claim Form (£50.00)

A medical certificate stating whether a child is fit or unfit for school attendance. It may also include the completion of school fees insurance claim forms, allowing parents to recover fees if the child is unable to attend school due to medical reasons.

ENG1/ML5 Seafarer Medical (£115.00)

A medical examination for individuals seeking to work at sea. The ENG1 is required for those working on UK-registered vessels, while the ML5 applies to smaller vessels. The examination ensures fitness to work in a maritime environment, focusing on vision, hearing, cardiovascular health, and physical condition.

Extraction from Records (£40.00)

This service involves extracting specific medical information from a patient’s records, typically for legal purposes, insurance claims, or supporting documentation for benefits. The extraction is done in accordance with data protection laws.

Firearms (£50.00)

Completion of a medical form for individuals applying for or renewing a firearms licence. The GP will assess mental and physical health, reviewing any history of mental illness, substance abuse, or other issues that may affect the individual’s suitability to hold a firearms licence.

Fit to Fly/Sports (£50.00)

A medical certificate confirming that an individual is fit to travel by air or to participate in sports activities. The GP will assess overall health and any conditions that may impact fitness to fly or engage in sporting activities, focusing on cardiovascular and respiratory health.

Forms (One Page) (£30.00)

Completion of brief, one-page forms, such as medical declarations for insurance, work, or travel purposes. This fee covers the GP’s time to assess the information required and complete the form.

Freedom from Infection Certificate (£30.00)

A certificate confirming that an individual is free from infectious diseases, often required for work, travel, or education purposes. The GP will assess the patient’s health and any relevant test results to issue this certificate.

Gym Membership Cancellation (£30.00)

Medical evidence is sometimes required to cancel a gym membership without penalty due to health reasons. The GP assesses the patient’s condition and provides documentation confirming that they are no longer able to use the gym facilities.

HGV Medical (£160.00)

A comprehensive DVLA medical required for those applying for or renewing a licence to drive heavy goods vehicles (HGVs). The medical includes checks for vision, cardiovascular health, and other factors that may affect the ability to drive large vehicles safely.

Healthcare Practitioner Questionnaire (BUPA) (£50.00)

A medical questionnaire required by BUPA for insurance or treatment purposes. The GP completes the questionnaire based on the patient’s medical history and current health, providing the necessary information for BUPA’s approval process.

Insurance Claim (Holiday etc) (£60.00)

Completion of medical forms required for travel or holiday insurance claims. These forms may be needed when a trip is cancelled or cut short due to illness. The GP provides information about the medical condition that affected the patient’s ability to travel.

Insurance Form (£95.00)

A more detailed medical form for insurance claims, often required by life, health, or travel insurance providers. The form includes a comprehensive review of the patient’s medical history and current health, sometimes accompanied by a physical examination.

Medical Fitness Certificate (£30.00)

A certificate confirming an individual’s fitness for a particular purpose, such as employment, education, or participation in sports. The GP will assess general health and provide a statement confirming fitness based on the findings.

Medical Treatment Form (BUPA) (£50.00)

A form completed by the GP for patients undergoing medical treatment covered by BUPA. The form provides details of the patient’s medical history and confirms suitability for the proposed treatment.

Photocopy Per Sheet (£0.50)

A service for patients who require photocopies of their medical records or other documents. Each sheet incurs a small administrative fee of £0.50.

PIP (Personal Independence Payment) (£33.50)

Completion of forms related to Personal Independence Payment (PIP), a government benefit for individuals with long-term health conditions or disabilities. The GP provides information on how the condition affects the patient’s daily life and ability to work.

POA (Power of Attorney) (£50.00)

A medical assessment to determine whether an individual has the mental capacity to grant Power of Attorney (POA). The GP assesses cognitive function and overall mental health to ensure that the patient is capable of making informed decisions.

Police Application Form (£30.00)

A medical assessment required for individuals applying to join the police force. The GP reviews the applicant’s physical and mental health to ensure they meet the necessary fitness standards for police work.

Pre-Employment Assessment (£30.00)

A medical assessment required by employers to determine an individual’s fitness for a job. The GP assesses general health and any conditions that may affect the patient’s ability to perform work-related tasks.

Pre-Treatment Form (Laser Hair Removal) (£50.00)

A medical form required before undergoing laser hair removal. The GP assesses the patient’s skin condition, overall health, and any contraindications to ensure the procedure is safe and suitable.

Private Consultation (No Fixed Time) (£100.00)

A private GP consultation for patients who wish to discuss health issues outside the scope of NHS services. This service allows for in-depth discussions without time restrictions, providing flexibility for the patient’s needs.

Private Prescription (Including Malaria) (£30.00)

Issuance of a private prescription for medications not covered by the NHS, such as anti-malarial drugs required for travel. The GP assesses the patient’s health and travel plans before issuing the prescription.

Private Sick Note (£40.00)

A medical certificate confirming a patient’s illness and inability to work, which may be required by employers for extended sick leave or insurance purposes. The GP assesses the patient’s health and provides a note detailing the condition.

Proforma Report No Examination (£50.00)

Completion of a medical report based on the patient’s medical records without a physical examination. This report is often required for insurance claims, legal purposes, or employment.

Proforma Report No Examination (Pre-Employment & Employment) (£100.00)

A more detailed proforma report required for pre-employment or employment purposes. This report is based on the patient’s medical records and does not require a physical examination.

Sports Medical No ECG (£75.00)

A medical examination required for participation in sports, excluding an ECG test. The GP assesses general fitness and health to determine whether the patient is safe to engage in athletic activities.

Sports Medical with ECG (£100.00)

A comprehensive medical examination for sports participation, including an ECG test to assess heart function. This service is often required for high-intensity sports or professional athletes.

Statement of Fact (£30.00)

A medical statement confirming specific facts about a patient’s health or medical history. This statement is often required for legal or insurance purposes.

Letters (To Whom It May Concern/Support/Housing/Third Party) (£35.00)

Personalised letters addressing specific needs, such as support for housing applications, third-party requests, or immigration purposes. The GP writes the letter based on the patient’s medical condition and the requirements of the requesting body.

Taxi Medical (£160.00)

A DVLA-compliant medical required for taxi drivers to obtain or renew their licences. The examination includes a review of the patient’s medical history, vision, and overall health to ensure they meet the fitness standards for driving professionally.

Taxi Medical 30 Minutes (£175.00)

A more detailed taxi driver medical that takes 30 minutes, including a thorough assessment of the patient’s physical and mental fitness for professional driving.

Taxi Medical 45 Minutes (£250.00)

A 45-minute taxi driver medical, offering a comprehensive evaluation of physical and mental fitness, suitable for those with complex medical histories or health conditions that may affect driving.

Taxi Medical 60 Minutes (£325.00)

A 60-minute taxi driver medical, providing the most detailed assessment of fitness to drive. This examination is typically required for drivers with more complicated health conditions that may impact their ability to work.

Vaccine Certificate (Including Childhood Vaccines) (£30.00)

A certificate confirming a patient’s vaccination status, including childhood vaccines. This document is often required for travel, school, or employment purposes to verify that the individual has received the necessary immunisations.
